Requires an available character slot. Make a new character, leave GTA Online, then go back online, and it will ask if you want to change your character's appearance. On that screen, sign out of PSN. Once you're in singleplayer, pause, go to Online, Choose Character, then select the character whose appearance you want to change, and you'll have the option to do so.

They did. Did the trick to re-design my character, recently. Before, his age was set to 40, and he looked suitably middle-aged. After, when I returned his age to 40, he looked like Ace's character. Had to drop him to 39 to make him look like he did before.
